Rural Indian Centre's experience with Teleradiology- "Teleradiology Providers" An Indian Pioneering Experience

Now that the old debate versus a Radiologist on hand verus a remote teleradiologist seems to be getting over and the role of teleradiology is increasingly getting defined. Here are a few cases where our radiologists from Teleradiology Providers read emergency cases sent from a remote rural centre in India where finding a cross sectional imaging trained radiologist was difficult. We further emphasize that most of the cases were blunt trauma (as it was located near the highway) where immediate reports were given and clinical management was affected by the reports given by our radiologists. This is one of the first report from an Indian setting. We from Teleradiology Providers further emphasize the role of teleradiology in country like India is unmatched. In our country we have a divide between the rural and urban practise and finding a well trained specialist is almost impossible to find in rural settings. With increasing availablity of Internet and phones remote specialist radiologist can easily become a part of team.

In our experience we included telephonic discussions with the operating surgeon and the CT technician so that we could discuss the protocol and surgical approach. At no point did anybody from the hospital feel that our radiologists were not easily available. Teleradiology Providers is a part of the team.

Intramuros: A Nostalgic Walk to the Old City

The walls of the Old City. During the Spanish times, Intramuros was the old Manila, sorrounded by a moat and massive walls, a citadel in itself.

The San Agustin Church was the only structure in Intramuros that survived the bombing of the Americans during the liberation of Manila from the Japanese in 1945.

The intricately decorated massive doors of the San Agustine Church. The lady seller was hoping I'd buy one of her rosaries in exchange for her posing in my picture...Okay, one rosary please...

This is the Garden of Father Blanco, as viewed from one of the tiers of the museum church. Father Blanco, an augustinian botanist, cultivated this magnificent garden in the 1860s. He published an illustrated three-tome book called Flora de Filipinas, which catalogues the diffrent flowers found in the Philippines. I wonder if Father Blanco knew the scientific name of Chicharon Bulaklak?

The entrance to the Fort Santiago. This was destroyed during the Seconfd World War, an American tank forced its way to the entrance, damaging the gate. I hope that tank was blown apart by the Japanese defenders.

This is what you'll see upon entering the gate. Now, that isn't boring...you have to get inside to get more interesting views.

An abandoned edifice that once saw glorious days during the Spanish times. it's actually called the Aduana or Customs House. Now only ghosts inhabit this building...as well as some beggars....

Another view from the side.

The Pasig River viewed from inside the Fort.

The olden tiled-tunnels that leads to the interior of the Intramuros.

The Manila Cathedral

Monument to Fernando de Magallanes, discoverer of the Philippines, died in the historic Battle of Mactan, killed by Lapu-lapu. I bet Magallanes would not have been killed if he asked Lapu-lapu to serve some fried danggit instead of fried Lapu-lapu

A historical marker to commemorate the Galleon trade between Manila and Acapulco. A sleeping man doesn't mind being photographed......because he was sleeping...

It's time for me to leave...a Jeepney is a convenient way to get me back to my house..It's time for me to cook some sinaing...

The Homeless

A homeless family sleeps in the bangketa in Chinatown, Binondo. Sights like these are common in Metro Manila. The little children are prone to disease, and they frequently just go to sleep to forget their hunger.

Young and Beautiful Kelsey Smith is Dead

Ever since I read from MSN news that Kelsey Smith, 18 years old, went missing last June 2, 2007, from a parking lot in Kansas City, I followed the news development of the massive search to find her, hoping that she would be found alive and fine.

This morning, when I opened my MSN news page, a headline said that a body bearing strong resemblance to Kelsey's was found by police, some 20 miles from the parking lot where she was last reported seen.
It was later confirmed by police that it was indeed the body of Kelsey Smith, who was just a fresh graduate from High School five days before her disappearance.
Apparently, on June 2, 2006, at about 7:00P.M., Kelsey went to a Target store to buy some gifts and wrappers for the graduation party she and her boyfriend were about to attend at 7:30. The boyfriend was waiting at her home so they can go together to the party. Kelsey never arrived.
A surveillance camera from Target store shows a young man with goatee entering the store a few minutes after Kelsey's. Kelsey left the store at about 7:10pm. She was holding the present and some long rolled wrapper. Then the camera captured the young man leaving the store shortly afterwards. The police were puzzled that the young man did not buy anything.
Kelsey went straight to her car, an old 87 Buick parked in the Target parking lot. It was there that she was last seen.
Apparently, it was the habit of Kelsey to call her parents whenever she will be late or going somewhere. Her parents were alarmed because it was not Kelsey's personality without calling when being late.
Concerned that something might have happened, Kelsey's family went to search for her that very night. Two hours later Kelsey's grandparents found her 87 Buick near the parking lot of Macy's, near Target. But Kelsey was not in her car.
Examining the footages of the surveillance camera in Target store, the police spread word that the young man is a "person of interest", although they haven't called him a suspect, just that he might know something or might serve as witness about Kelsey's disappearance.
As I write this blog, a man bearing strong resemblance to the young man in footage has already been arrested--thanks to anonymous tip to the police. I wish that it would be the starting point of solving this heinous and senseless crime.
In a news conference, Kelsey's distraught father, Greg Smith--a 16-year veteran policeman-- thanked all the people who helped her find her daughter.
For more info about Kelsey Smith and her beautiful life..please click here...
I condemn this senseless murder, as all of us should. What a promising young life has ended!
I extend my prayersand condolences to the family and friends of Kelsey Smith.

Post traumatic Spinal Cord Edema-Case Report

This is case of acute spinal cord edema in case of road traffic accident. Peculiar thing about this case was there is no vertrebral inury, only cord edema. Case submitted by Dr (Col) Murthy Sr Consultant Teleradiology Providers

MR imaging of recent spinal trauma. J Comput Assist Tomogr 1987 May-Jun;11(3):412-7
"Proper therapeutic management of acute spinal trauma depends considerably on the assessment of the condition of the underlying spinal cord and the relative stability of the surrounding bony architecture. A series of 14 patients who were examined by magnetic resonance (MR) imaging following spinal trauma is presented. Comparison of information gained by MR with that obtained by conventional CT indicates that MR is useful in assessing spinal cord compression, traumatic disk pathology, and post-traumatic intramedullary abnormalities. Computed tomography is more accurate in detecting posterior neural arch fractures and in assessing the number of displaced fragments."
